Change request form – Version 1.10​

Beneficiaries of Sub-Measures 4.1, 4.2 and 4.4 are to note that, following the granting of an automatic extension of up to three (3) months for projects, effected through letter dated 17 August 20211 (with the exception of projects of National Dimension), any future projects deadline extension requests may only be considered if duly justified and in cases of extraordinary circumstances only. 
Under no circumstance may requests for extensions exceed the date of 1st September 2022. 
As per previous notification, in case of all three sub-measures listed above, for project deadline extensions, a simplified approach is being adopted whereby a request by email sent to containing the following details shall suffice: 
Contract Number (which is written on the front page of the Grant Agreement)
Sub-Measure Number (e.g. 4.1, 4.2, 4.4)
Name of Beneficiary
Project Title (which is written on the front page of the Grant Agreement)
ID card number (in case of natural persons) or VAT number (in case of private legal persons)
A specific date up to when the extension is required
A substantiated justification for making this request (between 100 and 200 words)’
The MA retains the right to reject any request for extension.
